The Non-City Capital Project Task Force has produced Request Forms and supporting Documents to aid in the application process.

Links to the documents and detailed instructions can be found below. Please read the guidelines and bullet points for eligibility and requirements.


OVERVIEW

These guidelines, instructions and forms are only for NEW funding requested to support Capital Projects put forth by Non-Profit organizations.

Documents can be filled out in MS Word or printed.

Below please find links to the following supporting documents. Once again, these are only for Not–For-Profit capital requests.

Guidelines for Capital Request and Eligibility (PDF) - This is a detailed summary of the guidelines explaining how to apply for capital funding to support capital project requests.

Capital Request Form for Fiscal Year 2010
(MS Word)
– This is a fill-able form to request funding for a capital project for all not-for-profits, except organizations seeking funding for cultural capital projects.  This form must be submitted no later than March 5th at 5 p.m.

Short Form from the Department of Cultural Affairs (MS Word) – This short form must be completed by all not-for-profits requesting cultural funding including construction, renovations and equipment.  Exempt from filling out this form are cultural groups that are part of the Cultural Institutions Group (CIG) that are located on city property. The short form must be submitted and approved BEFORE the Capital Request Form for Fiscal Year 2010 can be completed.

Recommended Steps for Submitting Application:

  • Download these documents and forms. Right+Click (PC) or Control+Click (Mac) on the link and SAVE the document to your local hard drive.

  • Open the form in your Word Processing Program.

  • Fill in the fields within the form, being careful to follow all instructions within the forms and in the accompanying documents.

  • Save the form(s).

  • Send the document(s) as email attachment to:

  • Alternatively, you could click the document to open it, which would then launch your word processing program within the browser.

  • Fill in the form fields.

  • Click 'Save As' and save the forms to your local hard drive. Please add your organization's name to the document name so that we may identify your application.

  • Send the completed form as an email attachment to Mark Scott at the Borough President's office.
